They must have not included them in that specific version... In the 1.5 version of the game you can even see the level preview graphics for the rest of the levels

You probably would have gotten these levels if you ordered the game from the developers

According to this page, the guy also observed that the v1.5 shareware promised 10 worlds in the full version, but in v1.7 they now indicate 4 worlds, so I guess they had to scrap most of them: https://www.dosgamesarchive.com/download/twork/
